We understand that many people have never had an eye examination before or are nervous to have an examination as it is a sensitive part of the body. If you prefer that we do not do parts of the examination for any reason, please state your request during your appointment, and it will be noted in your medical records.
In order to provide the most efficient services to our patients we ask that you download the new patient forms prior to your appointment and bring them completed on the day of your examination.
It is often helpful if you bring a written list of all medications you are currently taking each time you see us for an appointment.
Most appointments will take between 30 minutes to one and a half hours. If you are scheduled for a dilated exam, you should expect to be in the office for at least one hour.
In order to stay on schedule and as a courtesy to our other patients, please make every effort to be in our office ready to be seen on time. If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours notice. This courtesy will make it possible to give your appointment to another patient.
Please note that your account will be billed 25 dollars if you miss your appointment or notify us later than 24 hours in advance of a cancellation. |
